A lot of affiliate marketing networks that are well established today are CPA networks. CPA stands for 'cost per action'.
The advertiser in this case pays you for each specified action. An action could be a purchase, or a form submission, and so on.

Trial offer: One bottle comes free, Just pay $3.98/S&H.
Once the consumer clicks and submits there credit card information, the company gives you your commission (In this case is $28.50).
